The foundation funding supports outreach and enrollment activities of the following Medical Coverage Collaborative partners: Axis Health System, Archuleta County Department of Human Services, La Plata County Department of Human Services and the La Plata Family Centers Coalition. All partners provide trained enrollment specialists at their sites to assist individuals and families in obtaining health insurance during open enrollment season from November through January, and throughout the year for those with a qualifying life event or who need assistance with applying for the state’s Medicaid program, Health First Colorado, or Child Health Plan Plus (CHP+).

The La Plata Family Centers Coalition has helped hundreds of individuals and families learn about and apply for health coverage.

Individuals who qualify for free or low-cost adult or child health insurance plans are encouraged to schedule a free consultation at one of the following locations: Durango 385-4747 (bilingual assistance available in Spanish), Bayfield 884-4747; or Pagosa Springs 264-5627 ext. 2.
