Health department receives grants for home services
San Juan Basin Health Department’s (SJBHD) Prevention and Population Health Division is striving to provide prevention and wellness services for Archuleta and La Plata County residents. One of the primary purposes of this division is to provide access, guidance, and advocacy for those who face barriers to healthcare. This is a public health priority and one that was identified in the department’s most recent public health improvement plan.
SJBHD is pleased to announce the continuation of these services during the next state fiscal year, through June 30, 2016:
. SJBHD receives funding to employ a nurse to work with the Temporary Assistance for Needy Families (TANF) staff at the Department of Human Services. TANF assists parents in ensuring children are immunized, assesses the need for family planning services, ensures children in the program are assessed for developmental delays and coordinates any necessary referrals, and helps parents set health goals with a care plan. SJBHD received $86,619 for this program.
. Nurse Family Partnership (NFP) is funded by the Colorado Department of Human Services (CDHS) and has provided services to Archuleta, La Plata, and San Juan Counties for over 13 years. NFP is a nurse home visitation program for low-income, first time mothers. SJBHD received $471,390 from CDHS for this program.
. SJBHD’s Single Entry Point Program (SEP) is funded by the Department of Health Care Policy and Financing (HCPF). The SEP Program establishes home and community based services within Archuleta, La Plata, and San Juan Counties with the goal of preventing nursing home placements for the elderly and disabled. SJBHD received $554,000 for this program.
