Registrants are asked to have the following information during the enrollment process: Residential address or addresses; parents or legal guardians phone numbers and email addresses; student demographic, health and medical information (name and date of birth should be entered as they appear on the birth certificate); emergency contact address and phone number; if applicable, previous school address and phone number.
